In order to claim cash assistance for students in crisis, the applicant must register online through a QR Code or by filling out the application form online. Please follow the steps below:

Step 3: Once all details are filled out, submit the form.
Step 4: Wait for instructions from DSWD officers for the date and location of how to claim the cash aid.

For those who registered online, the DSWD Social Worker will be conducting a screening or assessment on a payout day. Please bring VALID/AUTHENTIC/ORIGINAL and COMPLETE DOCUMENTS:
Valid ID (2 photocopies)- Up to date / not expired, not fake, government-issued with signature (except Philsys ID), and with picture.
Any one of them: (2 photocopies)
- Certificate of Enrollment or Registration; or
- School ID of the student
- Statement of account
- Any document from the school proving that the student has been enrolled
