An Affidavit of Support and Guarantee (ASG) is a legal document used to assure that a person will financially support or take responsibility for another individual, if the need arises. This type of affidavit is often required when applying for a visa, especially in countries where the applicant may not have sufficient financial means to support themselves during their stay. Requirements
All documents must be produced on A4-sized paper, including the original and two (2) photocopies.
The requirements for ASG are as follows:
- Filled but unsigned Affidavit of Support & Guarantee (ASG) form
- Sponsor’s valid and signed passport
- Sponsored individual’s valid and signed passport
- Sponsor’s residence card
- Proof of the relationship of the sponsor and the sponsored individual (PSA Marriage Certificate, Birth Certificate, etc.)
- Sponsor’s proof of financial capability to support the travel (ank statements, payslips, etc.)
- Sponsored individual’s flight details (if available)
- Self-addressed Return Envelope – Red Letter Pack (¥520)
- ¥3,500 (per set/document) processing fee
How to Apply for ASG at the PH Consulate Office in Osaka
Step 1: Download and print the Affidavit of Support & Agreement (ASG) form.

Step 2: Fill-up the information needed.

Step 3: Complete and compile the needed documents. Be sure to photocopy each document twice, to an A4 size.
Step 4: Visit the Philippine Consulate General in Osaka, Japan. The PCG office is open from Mondays to Thursdays -12:30 pm to 16:00 pm and Fridays – 9:30 am – 13:00 pm.

Step 5: Go to the Notarial window and submit documents for assessment.
Step 6: After assessing your submitted documents, the PCG personnel will call you for your signature.
Step 7: Sign the documents in front of the PCG personnel.
Step 8: The PCG personnel will review your documents.
Step 9: You will be asked by PCG personnel to pick between Regular and Express document release via mail.
- If you choose Express, you can receive your document the same day for an extra fee of ¥1,400, but it also depends on the signing officer’s availability.
- If you choose Regular processing and if all of the documents you submit are valid and filled, you will get your documents within five (5) to seven (7) working days after the application date. JP Post will be used for the document’s delivery.
Step 10: Proceed to the cashier to pay fees.
- Processing fees for each document are ¥3,500.
- If you would like express processing, there is an extra charge of ¥1,400 per document.
Step 11: Save a copy of your transaction slip and your official receipt.
Step 12: Wait until your ASG is released.

- Please be advised that the Consular Section’s notarial services DO NOT comprise writing legal documents or drafting wills, powers of attorney, deeds, or similar documents on behalf of the general public. The party using the legalization services is responsible for creating their documents according to their intended use and/or the specifications of the Philippine-based office to whom the notarized documents must be presented. - On the day of filing, the signatory must sign the document or documents for notarization in front of the PCG personnel.
- For a document to be notarized, the applicant’s true legal name must match the name on their valid ID. The applicant’s signature on the notarial document must match the signature on their passport and any other valid government-issued ID.
